About this role
Care Coordinator role providing care coordination primarily to First Nations clients with health-related goals within the Healthcare for the Homeless (ALERT) program at St Vincent’s Hospital Melbourne. The role supports improved health outcomes for people at risk of hospital presentation due to chronic disease or complex medical and psychosocial issues and partners closely with First Nations Services to ensure cultural safety and co-design. Permanent full-time position based in Fitzroy.
