Access Coordinator I

The Access Coordinator I ensures seamless progression for individuals entering services by guiding them through the intake process, supporting initial engagement, and rapidly addressing barriers to access. This role facilitates timely connections to appropriate services, acting as a bridge between initial contact and long-term care coordination. The position supports the Crisis/IRTS team and operates as a hybrid role combining remote and in-person work.
