Access Navigator

The Access Navigator provides information, assessment, and support to patients and families at entry and transfer points within Mental Health and Addiction Services. This role involves collecting clinical information, applying decision-making models, and directing referrals to appropriate care pathways based on clinical algorithms. The position requires collaboration with an inter-professional healthcare team to ensure client-centred and family-centred care.
