ACO Partner Performance Manager

The ACO Partner Performance Manager serves as the primary performance and relationship resource for assigned partner practices and facility operators participating in ACO REACH/LEAD and/or the Medicare Shared Savings Program (MSSP). This position translates financial, utilization, clinical, quality, attribution, and risk adjustment data into practical strategies that improve partner performance, strengthen care delivery, and support successful ACO outcomes. The Manager builds trusted relationships with practice leaders, physicians, advanced practice providers, facility administrators, and operator leadership; coaches partners through performance improvement; and establishes clear goals, accountability, and follow-through.
