ACT Therapist

The ACT Therapist provides community-based individual therapy, clinical, and substance use disorder support services to consumers with serious and persistent mental illness. This role involves joining a multi-disciplinary team to serve individuals at high risk for arrest or higher levels of care, utilizing the ACT Model to deliver intensive mental health and dual diagnosis therapy. The position focuses on crisis prevention, diversion planning, and coordinating care to reduce hospitalizations and incarcerations.
