About this role
The Activities Coordinator at AltaMed is a member of the PACE interdisciplinary team who supports the well-being of enrolled participants by coordinating and overseeing activity programming and contributing to care planning. The role focuses on promoting physical, cognitive, social, and spiritual engagement for frail or elderly participants within PACE centers. The position reports to the PACE Care Center Supervisor and collaborates with interdisciplinary team members to support participant-centered care.
