About this role

The Acute Care Transition Coordinator (ACTC) is responsible for coordinating bed management in the Acute Mental Health Inpatient Service, ensuring efficient bed flow to meet KPIs and government targets. The role involves supporting, mentoring, and training nursing staff while contributing to the recovery‑oriented model of care at St Vincent’s Mental Health.

Responsibilities

Coordinate bed flow and availability in acute mental health inpatient service
Monitor and achieve KPIs and government targets such as NEAT
Provide support, mentoring, training and education to students and early‑career nurses
Contribute to development and implementation of recovery‑oriented care models
Maintain compliance with vaccination and health‑safety requirements
