Addiction Counselor (SUDP)

The Addiction Counselor provides screening, assessment, individual and group counseling, and referrals for patients with or at risk for substance use disorders, using a patient‑centered, trauma‑informed, harm‑reduction approach. The role collaborates with care coordinators, therapists, behavioral medicine consultants, psychiatric and primary care providers to support patient wellness and recovery, and may serve multiple CVCH sites as needed.
