Admissions Coordinator

The Admissions Coordinator supports admissions operations for the Jewish Home & Rehab Center by coordinating safe and efficient transitions between care settings for incoming residents and patients. The role involves working closely with acute care hospitals, community stakeholders, and interdisciplinary teams to facilitate admissions, support census growth, and ensure exceptional customer service throughout the admissions and intake process. This position focuses on managing census and patient throughput, conducting tours, and maintaining professional relationships with referral sources.
