Adult Case Manager

The Adult Case Manager serves as a bridge between individuals and the support systems they need to thrive within the Community Support Services Program. This role focuses on empowering adults to navigate mental health services, achieve goals outlined in their Individual Recovery Plans, and connect with community resources. The position involves conducting needs assessments, coordinating care, and facilitating communication between clients, families, and service providers to promote recovery and community reintegration.
