About this role
An Advanced Practice Provider (NP/PA) role focused on building and delivering Devoted Medical’s Transitions of Care (TOC) virtual program, providing comprehensive post-discharge care during the 30-day window after hospital or skilled nursing discharge. The TOC APP works as part of an interdisciplinary clinical team to prevent readmissions through virtual assessments, individualized care plans, and coordination of follow-up services. This is a full-time remote role operating in PST/MST/CST hours.
