Advanced Practice Provider (NP/PA)

The Advocate Health Memory Center is seeking an Advanced Practice Provider to join its team and deliver compassionate, patient-centered care for individuals with Alzheimer's disease and other memory disorders. This role involves partnering with caregivers, collaborating with a multidisciplinary team, and managing an independent outpatient clinic with a high degree of autonomy. The position offers the opportunity to contribute to clinical, educational, and research initiatives within a nationally recognized dementia care program.
