Afterhours RN Care Manager

The Afterhours RN Care Manager provides high-quality, compassionate hospice care outside standard business hours, ensuring seamless coverage and continuity for patients and families. This role involves assessing and coordinating patient needs, conducting routine and urgent visits, and maintaining primary clinical oversight during nights, weekends, and on-call shifts. The position serves as an active member of the Interdisciplinary Group to coordinate care between physicians, families, and the hospice team.
