Afterhours RN Care Manager

The Afterhours RN Care Manager provides high-quality, compassionate hospice care outside standard business hours, ensuring seamless coverage and continuity of care for patients and families during nights, weekends, and on-call hours. This role involves assessing and coordinating patient and family needs, maintaining primary clinical oversight, and serving as an active member of the Interdisciplinary Group to ensure coordination between physicians, families, and the hospice team. The position also includes clinical administrative responsibilities, workflow auditing, and survey readiness support.
