Alumni and Family Coordinator

The Alumni and Family Coordinator supports alumni and family engagement efforts across their assigned region while being based onsite at an integrated care facility. This role partners with patients and families throughout the continuum of care, from admission through discharge and into sustained recovery. The Coordinator facilitates aftercare planning, supports family engagement efforts, and strengthens connections with alumni and community resources.
