About this role
The Ambulatory Care Clinical Coordinator (pharmacist) provides oversight of assigned areas such as Care Coordination or Network Liaison to ensure safe, efficient, and cost-effective medication use across the health system. The role partners with multidisciplinary teams to support the patient-centered medical home and expand pharmaceutical care across primary care clinics, community pharmacies, and other settings. This is a 9-month temporary position.

Qualifications
