Ambulatory Care Pharmacist

The Ambulatory Clinical Pharmacist provides integrated, patient- and prescriber-focused clinical pharmacy services to improve medication adherence and promote evidence-based therapy management across disease states. The role is remote and partners with onsite hybrid teams to deliver virtual assessments, clinical education, and coordination of care under approved protocols. The pharmacist supports quality improvement, regulatory compliance, and optimization of medication use while participating in clinical outcome initiatives and interdepartmental collaboration.
