About this role
The Ambulatory Clinical Pharmacist provides integrated support to patients and prescribers to improve medication adherence and advance evidence-based therapy management across disease states. The role works with interdisciplinary teams and a hybrid ACP team to support clinic-based pharmacotherapy services, ensuring safe, effective, and cost-conscious drug therapy. The pharmacist performs in-person and virtual assessments, supports clinical programs and quality initiatives, and helps implement workflow improvements to enhance patient care.
