Ambulatory Care Pharmacist

The Ambulatory Clinical Pharmacist provides integrated, patient-focused support to improve medication adherence and promote evidence-based therapy management across disease states. The role is primarily remote and collaborates with onsite clinical teams to deliver phone and virtual assessments, education, and clinical oversight per approved protocols. The pharmacist helps identify, resolve, and prevent medication-related problems while supporting quality improvement and regulatory compliance initiatives.
