Ambulatory Care Pharmacist

The Ambulatory Clinical Pharmacist provides remote, integrated clinical support to patients and prescribers to improve medication adherence and ensure evidence-based pharmacotherapy across targeted clinics. The role partners with onsite and hybrid clinical teams to deliver therapy management, clinical assessments, and patient education via phone or virtual platforms. The pharmacist also supports quality improvement, regulatory compliance, and prior-authorization activities to optimize medication outcomes and cost-effective care.
