About this role
The Ambulatory Clinical Pharmacist provides integrated, patient-centered medication management to improve clinical outcomes and medication adherence for assigned clinics. This role supports physicians and interdisciplinary teams by delivering virtual assessments, therapy management services, and evidence-based recommendations. The pharmacist works remotely while collaborating closely with onsite clinical teams to optimize pharmacotherapy across disease states.
