Ambulatory Case Manager

Astiva Health Inc. is seeking a highly energetic and collaborative Ambulatory Case Manager to work within an interdisciplinary team and facilitate patient care plans across the continuum of care. The role focuses on ensuring appropriate utilization management, care coordination, and clinical documentation in accordance with the Model of Care and CMS regulations. The Ambulatory Case Manager will assess member needs, develop comprehensive care plans, and coordinate with internal and external partners to achieve quality clinical and financial outcomes.
