Ambulatory RN

The Ambulatory Care RN performs Medicare Annual Wellness visits and provides quality triage and patient care using strong assessment skills and point-of-care testing. The role involves collaborating with clinicians to assess and establish care plans for complex patients, triaging and coordinating care for acute and chronic disease management, and advocating for patient rights and values. The RN works closely with the interdisciplinary care team to improve quality, patient satisfaction, and contribute to the ongoing development of the practice and staff.
