Assertive Community Treatment (ACT) Therapist (4747)

Carolina Outreach is seeking a full-time Mental Health Therapist to provide community-based, in-home individual and family interventions to clients receiving Assertive Community Treatment (ACT) services. The role involves service coordination, crisis assessment and intervention, symptom management, and delivering evidence-based therapy to support clients' mental health and substance use disorder treatment. The therapist will collaborate with clients, families, and care teams to develop and implement person-centered plans while maintaining clinical documentation and regulatory compliance.
