The Bed Readiness RN Referral Coordinator manages patient referrals across post-acute settings to ensure timely, appropriate placement aligned with clinical needs and system capacity. This role serves as a central liaison between hospitals, care teams, and external partners to optimize patient flow and reduce length of stay while supporting safe transitions of care.

Responsibilities

Coordinate and manage incoming and outgoing referrals for post-acute placement
Assess patient clinical needs to identify appropriate levels of care
Serve as primary liaison between hospital teams and external partners
Monitor bed availability and placement timelines across the network
Track referral trends to identify opportunities for process improvement
