Behavioral Health Advocate

The Behavioral Health Advocate provides patient advocacy and care coordination for individuals with complex medical, behavioral, and social needs in the Eagle Valley Family Practice Clinic. Working under the direction of the LCSW, Clinic Director, and other providers, the role focuses on integrating patients into behavioral health services, managing high‑risk cases, and addressing social determinants of health through community resources and motivational interviewing.
