About this role
The Behavioral Health Case Manager II is a field-based clinical role delivering case management and hospital liaison services for members with complex psychiatric and substance use disorders in Manhattan (with occasional Brooklyn travel). The position performs assessments, develops and monitors individualized care plans, and coordinates internal and external resources to support access to appropriate, cost-effective care. Work combines in-person field visits, telephonic outreach, and occasional virtual or office attendance for meetings and training.
