About this role
The Population-based Community Behavioral Health Clinician at Boston Medical Center Health System provides integrated behavioral health support for patients with or at high risk for cardiometabolic conditions such as diabetes, hypertension, and obesity. The role is part of the Health Equity Accelerator and works within multidisciplinary teams to advance health equity and connect clinical care with community-based programs.
