Behavioral Health Consultant

The Behavioral Health Consultant provides brief, evidence-based assessments and interventions to improve patient functioning within a primary care setting. This role involves identifying, triaging, and co-managing patients with medical and behavioral health problems while delivering psychoeducation and behavioral change plans. The consultant collaborates with the integrated clinical care team to support both providers and patients through mental health consultations and crisis intervention.
