About this role
This role supports patients in a Residential Treatment Center by completing intake assessments and coordinating care planning and discharge planning services. The position serves as a key liaison between patients, families, clinical staff, and community resources to ensure continuity of care throughout treatment and after discharge. It is a weekday, full-time schedule (Monday–Friday, 8 AM–5 PM) within the Boys Town residential mental/behavioral health setting.
