About this role
The Lead Care Manager at Pair Team is a full-time role on the Enhanced Care Management team focused on engaging high-need individuals (including those with Serious Mental Illness, Substance Use Disorder, homelessness, or complex medical needs) within a whole-person, interdisciplinary care model. The role supports Pair Team's mission to improve access and outcomes for underserved Medicaid and Medicare populations. This position is primarily remote but includes regular community-based in-person visits 2–3 times per month.
