About this role
The Lead Care Manager is a full-time role focused on engaging high-need Medicaid recipients through a community-led primary care model. The role supports individuals with serious mental illness, substance use disorder, homelessness, and complex medical needs by building trust and connecting them to whole-person care and community resources. The position primarily works remotely with regular in-community visits 1-2 times per month.
