The Lead Care Manager is a full-time role supporting Pair Team’s whole-person, interdisciplinary care model to engage high-need individuals—particularly those with serious mental illness, substance use disorder, homelessness, or complex medical needs. The position is primarily remote with periodic in-community onsite engagement near the employee’s city.
