Bilingual Case Manager

The Bilingual Case Manager provides coordinated stabilization and time-limited case management support to English and Spanish-speaking clients exiting unsheltered homelessness or transitional shelter sites. Working within a collaborative team, the role focuses on ensuring rapid exits into housing and delivering intentional follow-up care, including crisis intervention, basic needs provision, and linkage to medical and behavioral health services.
