Bilingual RN Care Manager

The Registered Nurse Care Manager serves as a clinical resource within Titanium Healthcare's Enhanced Care Management team, providing comprehensive nursing care, medication reconciliation, and health education to members with complex medical and behavioral needs. This role involves coordinating care across various providers and community partners, conducting both telephonic and face-to-face assessments, and ensuring compliance with Department of Health Care Services standards. The position requires facilitating transitions of care, developing individualized care plans, and collaborating with an interdisciplinary team to improve health outcomes for individuals with chronic conditions.
