Birthing Families Navigator

The Birthing Families Navigator works with medical providers and care teams to identify potential clients and deliver supportive services to birthing families. The role involves outreach, advocacy, assessments, care planning, referrals, and home visitation to ensure clients receive comprehensive prenatal, postpartum, and pediatric care. This position supports Erie Family Health Centers' mission of providing high-quality, affordable care to diverse communities.
