Blended Case Management

This role provides case management services to behavioral health consumers, focusing on implementing and updating their Strengths Assessment Scale and Service Plan. The position involves maintaining clinical records, coordinating appointments, and linking consumers to essential social and medical services. The case manager also serves as a member of the on-call system and advocates for consumers and their families.
