BR Outreach Coord Clinical

The Community Outreach Team Member – Clinical supports the RuralCare Connect program by conducting community-based clinical screenings, outreach activities, patient education, and care navigation services in Burlington and Ocean Counties. This role involves participating in mobile health screenings, outreach events, and preventive care initiatives to improve access to care for rural and underserved populations. The position requires performing basic clinical screening services and supporting patient engagement, referrals, and follow-up coordination with clinical teams.
