The Cancer Navigator collaborates with hospital and clinic caregivers to educate patients and families on home care, palliative care, and hospice services, facilitating informed discharge decisions. This role serves as the primary communication link between patients, providers, and external partners, coordinating transfers to the Hospital at Home program. The position involves assessing patient needs, ensuring logistical readiness for home care, and providing comprehensive support throughout the transition process.
