Care Coordination Assistant

The Care Coordination Assistant provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams at Baptist Metro, facilitating coordinated, patient-centered care and smooth transitions for patients and families. The role involves assisting with discharge planning, maintaining accurate documentation in the electronic medical record, and coordinating meetings, referrals, and follow-up appointments. This full-time, day shift position requires 1-2 years of hospital experience, a high school diploma, Basic Life Support certification, and a valid driver's license.
