Care Coordination Manager - Norwich - Hybrid

The Care Coordination Manager provides comprehensive, person-centered care coordination to individuals with intellectual and developmental disabilities in New York State. The role oversees caseloads, develops Life Plans, and ensures access to medical, behavioral health, and community services while maintaining rigorous documentation standards. CCMs collaborate with interdisciplinary teams, families, and advocates to promote independence, eligibility for benefits, and consumer satisfaction, with the ability to telecommute.
