Care Coordinator - Bilingual (Spanish)

This role provides support to families with children facing complex behavioral, emotional, and mental health needs by utilizing a family-driven, strength-based, and community-based approach. The Care Coordinator engages, educates, and empowers families through the Wraparound process, creating care and safety plans while facilitating team meetings to ensure sustainable support. The position requires maintaining case loads, conducting assessments, and collaborating with local and statewide networks to resolve service access issues.
