Care Coordinator - Bilingual (Spanish)

This role provides support to families with children facing complex behavioral, emotional, and mental health needs by utilizing a family-driven, strength-based, and community-based approach. The Care Coordinator engages, educates, and empowers families through the Wraparound process, creating care plans and facilitating team meetings to sustain families in their homes. The position requires maintaining weekly contact, completing assessments, and collaborating with local and statewide teams to ensure access to necessary community services.
