Care Coordinator - Care Finder Program

Each is seeking passionate Care Coordinators to support older Australians experiencing vulnerability or isolation by navigating the aged care system and connecting them with vital community services. The role involves assertive outreach, building trust with clients and families, and establishing relationships with local government and voluntary organizations. Candidates will work independently in the community and collaboratively with colleagues to ensure clients do not fall through the gaps.
