The Living Room Center
The Care Coordinator, CCBHC, collaborates with the outpatient team to coordinate referrals and linkages, helping members navigate and access services. This role supports members through community outreach and case management to improve health and recovery outcomes while maintaining data tracking and reporting in alignment with CCBHC requirements.
The Living Room supports women and children in urgent need or at risk of homelessness, providing care, community, and a pathway to stability. Through comprehensive services, we help families build resilience and move toward lasting success.
