Care Coordinator, Disability Support Program

The Care Coordinator works under the direction of the Casework Supervisor to manage the intake, assessment, and financial eligibility for individuals with disabilities within the Disability Support Program. The role involves developing individual support plans, coordinating services, and managing cases to help vulnerable Nova Scotians achieve independence and security. This position requires collaboration with various agencies and support networks to meet the assessed needs of clients across their lifespan.
