Care Coordinator- Ellis County (The Heights)

The Care Coordinator position provides timely intervention and comprehensive, trauma-informed services to help individuals prevent decompensation and relapse. This role primarily serves an assigned county and requires strong organizational and clinical skills to ensure continuity of care and provide referral and resource solutions. The position involves complex services including screenings, assessments, crisis follow-up, and transitional services to connect individuals with mental illness and/or co-occurring disorders to appropriate care.
