Care Coordinator - Families FIRST

Terros Health is seeking a Care Coordinator for the Families FIRST program at their Stapley Rd Health Center in Mesa, AZ. This role involves working with families involved with the Department of Child Safety to overcome substance abuse barriers and achieve family reunification and self-sufficiency. The Care Coordinator will collaborate with DCS, clinical teams, and other providers to coordinate services, conduct monthly meetings, and empower families with resources and skills.
