Care Coordinator - Pathlight HOME

IMPOWER is seeking a Care Coordinator to support behavioral health services for individuals served through the Pathlight program. This on-site position plays an important role in helping individuals access and stay connected to care by coordinating clinic visits, outpatient therapy, medication needs, referrals, and telehealth services. The role serves as a primary point of contact for individuals served and works closely with clinical providers and community partners to ensure timely access to services and continuity of care.
