Care Coordinator

This role directs patient care in an outpatient setting by coordinating with staff, providers, and the Nurse Manager to ensure clinical policies are met and patient needs are addressed. Responsibilities include managing high-risk populations, facilitating daily care huddles, tracking referrals and test results, and educating patients on disease management and community resources. The position also involves maintaining accurate nursing documentation, ensuring proper follow-up after hospital visits, and removing barriers to care for the patient population.
